My birth day and my birth month differ by a single digit, so I've started writing down (e.g.) "May 4th, 198X" on a lot of forms to minimize the confusion when it'll matter, like at a doctor's office & such.
But in my personal life, and just about anywhere else, I stick to YYYY-MM-DD as the only sane option.
